Prevention of post-ERCP pancreatitis using pancreatic duct stenting in difficult cannulation patients with calcular biliary bbstruction

Background: Post-ERCP pancreatitis (PEP) has been the most frequent complication of Endoscopic Retrograde Cholangiopancreatography (ERCP). It documented to happen after 5–30% ERCP procedures. PEP is new or exacerbated abdominal pain associated with a serum lipase amylase concentration which more than three times upper limit normal at 24 hours post-ERCP, requiring least two days prolongation planned admission. Following pancreatic sphincterotomy, stenting commonly used objective decreasing both early restenosis and post-ERCP pancreatitis. Aim work: Evaluation technique as preventive measure against post in calcular obstructive jaundice patients difficult cannulation. Patients methods: Forty Egyptian biliary obstruction cannulation who are risk developing enrolled prospective randomized controlled comparative study. Group A: 20 manipulation duct by guidewire without stent insertion; B: insertion.Results: The had reduced significantly CBD 20?% (in group B) compared 60?% (group A).Conclusion: Prophylactic simple easy procedure that showed be efficient cases patients. endoscopists should trained practice safely.
